Waiver 1. By signing this waiver I (applicant and parent/guardian) have read, understood and agree to the conditions and rules of this contest. 2. I understand that prior to entering this contest a waiver must be signed by me (applicant) and by my parent or guardian. 3. I understand and agree that my (applicant’s) contest entry, upon submission, becomes the property of The Lloydminster Community Youth Centre. 4. I, (applicant) and my parent/guardian understand and agree that once the (applicant’s) video is submitted it will be uploaded on the Youth Centre YouTube page. 5. I understand and agree that should the applicant’s video be the winning video that it will be aired at schools as well as on TV.
